Wisdom Teeth That Makes Pain<\/a><\/h3>
What are wisdom or impacted teeth? Impacted teeth are teeth that cannot or cannot erupt into their normal functional position due to resistance from adjacent teeth, bone or surrounding pathological tissue. This situation can lead to pathological conditions that require further treatment. Impacted teeth are divided into two conditions, namely full impaction or total impaction (complete impacted) and partial impaction. \n\n The American Association of Oral and Maxillofacial Surgeons (AAOMFS) in their electronic book entitled Wisdom Teeth states that 9 out of 10 people have at least one impacted tooth (Ebook Wisdom Teeth, AAOMFS, 2021). The condition of impacted teeth is a common condition, which ranges from 0.8-3.6% of the total population in general. Impacted third molars have the greatest prevalence, which is between 16.7% and 68.6%, and it is estimated that around 65% of the world's human population has at least one impacted molar (Silvestri and Singh, 2003; Goyal et al., 2016). The most frequently impacted teeth were the mandibular and maxillary third molars, followed by the maxillary canines and mandibular premolars. This is because the third molars are the last teeth in the eruption sequence, resulting in a lack of room for eruption. \n\n \n\n Impacted teeth often make it difficult to maintain oral hygiene because of their position which cannot be reached by a toothbrush, which can cause adverse effects in the long term. Impacted third molars are the most common cause of caries second molars due to food retention. The excess of soft tissue over the coronal part of the partially erupted impacted tooth often causes gum inflammation due to the difficulty in cleaning the pseudopocket area that is formed, thus becoming a place for the development of cariogenic bacteria and periodontal pathogenic bacteria found in the oral flora. Inflammation that appears in the soft tissue around impacted teeth is called pericoronitis. This condition can cause an infection, which, if left untreated, can spread to the head and neck area. Impacted teeth can stimulate the occurrence of cysts and other pathological forms, especially during the formation of teeth. Tooth seeds that experience interference with the formation process so that they become imperfect, can cause primordial cysts and follicular cysts. Another pathological condition that can arise as a result of impacted teeth is an odontogenic tumor. The frequency of odontogenic cysts and tumors arising from impacted third molars is approximately 3%. Endometriosis is a benign, estrogen-dependent inflammatory disease characterized by the presence of ectopic endometrial implants. The most common predilection for endometriosis is the ovaries, followed by the anterior/posterior cul-de-sac, broad and uterosacral ligaments, uterus, fallopian tubes, sigmoid colon and appendix. Endometriosis is a chronic inflammatory disease in women of reproductive age and can cause pain and infertility. \n\n \n\n The pathogenesis of endometriosis can be explained by several theories such as retrograde menstruation, immune changes, coelomic metaplasia, metastatic spread, stem cells, and the role of genetics. \n\n \n\n In the theory of retrograde menstruation proposed by Sampson in the 1920s, it was proposed that endometrial tissue is transported retrograde through patent fallopian tubes into the peritoneal cavity. Endometrial cells then attach to peritoneal mesothelial cells, form a blood supply, proliferate and produce endometrial implants. \n\n \n\n In the 1960s, Ferguson proposed the theory of coelomic metaplasia according to which the peritoneum contains undifferentiated cells that can differentiate into endometrial cells. \n\n \n\n Meanwhile, in the theory of pathogenesis of immune changes, it is stated that women with endometriosis have changes in the immune system that prevent them from clearing reflux cells/endometrial fragments that appear in retrograde menstruation. This may explain why some women with retrograde menstruation develop endometriosis while others do not. Cell-mediated immunity is thought to be deficient in patients with endometriosis, as a result of which leukocytes cannot recognize that endometrial tissue is not in its normal location. There are also studies showing decreased cytotoxicity in endometrial cells secondary to impaired NK cell activity. Once endometriosis has formed, the body's immune system also potentiates the development and increases the severity of endometriosis. In women with endometriosis, there is an increased number of leukocytes and macrophages in and around the endometrial implant and in the peritoneal fluid. These cells secrete cytokines and growth factors (IL-1,6 and 8, TNF, RANTES, VEGF) into the peritoneal environment, which in turn recruits capillaries and surrounding leukocytes leading to proliferation of endometrial implants with increased vascular supply. \n\n \n\n Endometriosis causes fertility problems through mechanical disturbances such as adhesions, impaired release or retrieval of oocytes, impaired sperm motility, irregular myometrial contraction disorders, impaired fertilization and embryo transport, and inflammatory disorders. \n\n \n\n The gold standard for diagnosing endometriosis is laparoscopy. On ultrasound, the picture of the endometriosis cyst shows a ground glass appearance. To determine the degree of endometriosis, the endometriosis infertility index developed in 2010 can be used which has an association with pregnancy rates (Figure 1). \n\n \n\n \n\n Figure 1.
